NEW PATIENTS

Permanent Residents

To register as a new patient you must live within our Practice boundary area​ (see below image).

 

We will also require current identification; please bring the completed forms below along with two of the following (preferably one photo and one address identification).

 

Please complete one the relevant forms below along with the GMS1 Form - all forms should be completed in black ink.

 

The information supplied is completely confidential and will be used to form your patient records.  The forms will be scanned on to your electronic patient health records.

 

The forms may only be completed by the patient, or a parent/carer in the case of a child. 

 

Please return completed forms to reception, we will then write to you to inform you who your registered GP is and you will be issued with a New Patient Information Pack.

Once you are registered as a patient you will receive a copy of our welcome book which has all the useful information you will need to know about our surgery.  You can also download a copy of our welcome book by clicking here.

Temporary Residents

You can register as a temporary resident if you plan to live near Hucclecote Surgery for up to 3 months.  This may be because you are living away from home for work, study or on holiday.  You'll remain registered with your permanent GP Surgery.  We will pass details of any treatment you have here onto your permanent GP Surgery and they will add the information to your medical records.
