End of Life Survey
As part of the Clinical Commissioning Groups End of Life Care Strategy, Healthwatch Gloucestershire is looking at what end of life support is available for individuals over 18 years old both locally and nationally. This support is non-clinical support; not information about illnesses or conditions but other types of information that would support patients or families through the end of life. We want to hear from Gloucestershire residents only, who have accessed support around end of life care or who would be looking at accessing this information. End of life patients are individuals ‘approaching the end of life’ when they are likely to die within the next 12 months.
The information gathered during the evaluation will help to understand what is available to support people locally and nationally, identify gaps and raise awareness of this support to those that need it as part of the End of Life Care Strategy and improve end of life care.
