If you have just moved in to the Area and are not already registered with a local doctor, you may be able to register with us.

If you have a medical card, please complete it and call into the surgery where the receptionist will happily register you. If you do not have a medical card come into the surgery and you will be handed a form to complete. We will need your National Health Number before we can register you.

Once we have either the card or the form, the receptionist will tell you about the surgery and invite you to make an appointment for a New Patient Check.

If you are on NO MEDICATION the 'check' can be carried out by the Practice Nurse.

If you are TAKING MEDICATION the 'check' will need to be completed by the Doctor so that he/she can discuss your illness and medication with you, and enter it onto the computer.

You will also be asked to complete a quesionnaire and to bring it with a sample of urine when you come for you New Patient Check.
